Thank you for contacting us.
There are two ways for you to rename your form.
One is from the My Forms section, where you can hover over your form and go to the Show More-> Rename option. The other option is to change its name directly from the top-left corner of the form’s editor.

Please send me instructions on how to edit the name of my form – I used a template and now struggle to edit the name of the form. thank you
