This can be easily achieved by enabling a password for your online form. You can set this up from the forms Security section.
Once you enable this feature and set up a password, the users will be met with a message to add a password and only after they add it correctly they will be able to see the form and send their submission.

Hi, I wanted to create a form with an access code so that only users who enter a code can fill out the form. How would I do that?
