Integrating forms and surveys with Facebook is very easy to do, due to the 123FormBuilder application for Facebook. Access it on the publish page (it will be installed automatically).
Choose the form that you want to post and the Facebook page where you want to post it. Afterward, you can update your page/form association or access your form editor directly from your Facebook page.
1. In your form builder account, go to the Publish section of your form and hit the Facebook tab on the left. From there, click on the Install app button, below the video tutorial.
2. In the lightbox that appears, select the form you want to add and on which page (if you have more than one per Facebook account).
3. Click on Add to publish the form on your page and select to which page will the 123FormBuilder tab be applied. Hit Add Page Tab to finish.
A confirmation message will appear on the screen along with a Click here link to see the published form.
The message in orange, above the form, is shown only to you and not to other people who visit your page and complete the form.
Facebook has a requirement that a Facebook page needs to have a minimum of 2000 fans to be able to use and publish custom tabs.
