REGISTRATION FORM

I hereby register my child at Stars Dance Studio for the 2020-2021 Dance Class/Classes selected below. The annual registration fee is $35 per dancer for recreational dance. This can be paid through Venmo @Stars-Dance-Studio or through PayPal on our website. Registration fees must be paid in order to reserve your space in your chosen class.
Limited space is available for each class.

RELEASE: I hereby release Stars DanceStudio and all staff officials from all claims of damages or injury suffered by the registered student in connection with their association with Stars Dance Studio. This includes my heirs who may not act in my behalf. I have read and understand the terms of the above contract and the program information sheet and agree to abide fully by its terms.
