I hereby register my child at Stars Dance Studio for the Summer Dance Camp selected below. The fee for each summer camp is $60 and Wednesday Summer Classes $45 this can be paid in the studio office, venmo or by clicking on the Pay Now Paypal button found on the Summer Camps tab on our website. Fees must be paid in order to reserve your spot in chosen camp or class. Limited space is available for each camp.
RELEASE: I hereby release Stars DanceStudio and all staff officials from all claims of damages or injury suffered by the registered student in connection with their association with Stars Dance Studio. This includes my heirs who may not act in my behalf. I have read and understand the terms of the above contract and the program information sheet and agree to abide fully by its terms.
