I, (Participant’s name), release Times Square Church, its agents, assigns, employees and volunteer assistants from any and all liability whatsoever arising out of injury, sickness, or damage which may be sustained during the course of any activity with Times Square Church.

I, (Participant's name), consent for the director or properly appointed staff member or volunteer leader of Times Square Church to secure the administration of medical treatment or medication. I further consent to the performance of such treatment, anesthetic, and operations as, in the opinion of the attending physician, is deemed necessary.
List medication Participant must take, the dosage, and time intervals. Please include a doctor's note verifying the condition and the treatment.

I, (Participant’s name), understand that it is my responsibility to provide for my accident and health coverage while participating in all activities with Times Square Church. I understand that my health insurance will be the primary insurance for any accident or medical claim, and that Times Square Church is under no obligation to pay for such care. I further understand that I am financially responsible for any treatment, prescriptions, or hospital care obtained for myself. I consent to the release of this information as needed.

I understand and agree that:
  • While I’m on a TSC Missions Trip or in training, I may appear in photos and/or video.
  • Times Square Church or a related ministry may use those photos or video for any purpose.
  • TSC may select, position, caption, and edit the photos or video in its sole discretion.
  • I will not be consulted or notified if the photos or video appear.
  • If I submit photos or video to TSC Missions from the trip or training, I agree to allow TSC to use my photos or video to promote TSC or a related ministry, and agree that I will not be paid for any use. 
  • I release any claims against TSC for the use of photos or video in which I appear or which I submit for use.

I agree to make all payments on a timely basis according to the required payment schedule. I understand that the trip balance must be paid in full by the final payment deadline or I could forfeit my place on the trip. 
  • The $250 deposit that is paid when you are accepted to a trip is an administration fee and is non-refundable and non-transferable.
  • Should you find that you are unable to travel with us on this trip for any reason, your funds can be transferred to:
    • Where the need is greatest
    • Tithes and offerings
    • Someone else’s trip  
  • If you should exceed your goal in fundraising the balance will go into a TSC Missions fund that will help offset short-term mission expenses where the need is greatest.
  • Informing friends and family about this policy is my responsiblity 
I agree to turn in all paperwork on time and fulfill all training requirements listed on the acceptance letter as well as any additional training and prayer requirements designated by TSC staff or my team leader.

Statement of Faith
1. The Bible is the inspired and only infallible Word of God.

2. There is one true God, eternally existent in three persons: God the Father, God the Son, God the Holy Spirit.

3. In the deity of our Lord Jesus Christ, in His virgin birth, in His sinless life, in His miracles, in His vicarious and atoning death, in His bodily resurrection, in His ascension to the right hand of the Father, in His personal future return to the earth in power and glory to rule for a thousand years.

4. In the blessed hope, the imminent return of Christ for overcoming believers.

5. In the fall and sinfulness of man and that the only means of being cleansed from sin is through repentance and faith in the redeeming blood of Christ.

6. Regeneration by the Holy Spirit is absolutely essential to personal salvation.

7. Divine healing is available through the redemptive work of Christ on the cross.

8. The Baptism of the Holy Spirit, according to Acts 2:4, is given to believers who ask for it. The Holy Spirit in a person’s life is evidenced by changes in his life, including a renewed love for God and people, a commitment to the Scriptures and holy living. The spirit filled believer has the capability of speaking with new tongues whether or not he or she chooses to exercise this gifting.

9. The Church of Jesus Christ is the universal, spiritual body of believers from every tribe, tongue, kindred and race of peoples, and is indwelt by God through the Holy Spirit and divinely empowered to fulfill ministry and her Great Commission on earth.

10. In the sanctifying power of the Holy Spirit by whose indwelling the Christian is enabled to live a holy life.

11. In the resurrection and final judgement of both the saved and the lost, the saved to everlasting life and the lost to everlasting damnation.

12. In the new heavens and the new earth and the holy Jerusalem, the city of God, descending out of heaven and filled with glory of God.

13. Christian growth can only be realized by faith in the promises of God.

14. Marriage is God-ordained. “The Creator made them male and female, and said, “For this reason a man will leave his father and mother and be united to his wife, and the two will become one flesh” (Matthew 19:4-5). Marriage is to be an exclusive relationship, a lifelong faithful union between a man and a woman. This relationship between a husband and wife should parallel the relationship between Christ and the Church (Ephesians 5:23-30).

15. We practice two ordinances:
(1) Water Baptism by immersion after repenting of one’s sins and receiving the gift of salvation.
(2) Holy Communion (the Lord’s Supper) as a symbolic remembrance of Christ’s suffering and death for our salvation.
